Centennial Adultcare Center is an alternative or supplement to home care, assisted living, or nursing home care enabling our members and their families continued community-based living and providing respite for their caregivers.
Centennial Adultcare Center offers a full range of adult day health care programs both center based as well as in-home services to individuals with a wide variety of physical disabilities and/or cognitive impairment, including advanced Alzheimer's disease and other dementia's. We provide a stimulating social and supportive environment responsive to the needs of our members, and respite and supportive care to the caregivers through personalized professional care at reasonable costs.
Adult day health care programs offer highly structured routines that include daily physical and mental exercises, social activities, music, games, and cognitive stimulation designed to enrich the lives of our members while providing the much needed relief for the caregiver. We provide a nutritious lunch as well as morning and afternoon snacks and assistance with toileting and medications. Skilled nursing services are provided for ongoing health assessment and personalized care plans. Other services available include assistance with bathing and oral hygiene, beautician services (manicure, pedicure, haircuts and styling and more). Full transportation services are also available.
In-home services include; personal care, such as assistance with bathing and toileting, medication reminders and feeding assistance; homemaker services, such as cleaning services, laundry, shopping, and errands; respite care while a caregiver is absent; and companion care for someone in a hospital or facility where more supervision is necessary. All in-home services are available 24 hours/day, seven days/week including holidays.
Financial assistance may be available through the Veterans Administration, the Tennessee State Options Program, or through the TennCare CHOICES Program. Please contact us for more details.
